Update for those taking the train from Hamburg HBF (and not ALTONA)
There are 2 trains running to Elmshorn where you can change on the Nord-Ost-See Bahn (NOB). Either the RE (Regional Express) to Kiel or the RE to Flensburg via Neumünster. Both call in Elmshorn, where you can change for Itzehoe.
Unless you're lucky to get a direct one to Itzehoe of course!

From Lübeck Flughafen/Airport.
DB (Deutsche Bahn / German Railways)
Lübeck Hbf - Hamburg Hbf/Altona: 2x hour a train. Price: 12.95€
Lübeck Hbf - Itzehoe : 1x hour a connection, change in Hamburg. Price: 23.30€
If you stumble by chance on an IC connection (between Hamburg Hbf and Itzehoe) the price is 28.50€
Schleswig-Holstein minigroup ticket: 33.60€, but no IC/ICE trains allowed!
A minigroup is minimum 2, maximum 5!
From Hamburg Airport
S-Bahn Flughafen - Itzehoe: 3x hour a connection.€12.95 (€19.50 with IC Hamburg Hbf - Itzehoe)
