Germany: Phone House, D-Telekom, Vodafone shops etc. Don't ask for addresses, I haven't got an idea.

You can get them on Ebay too (if you'd prefer to have them before you go). The guy I bought mine from a few years back even let me choose how much money to put on it and the card itself wasn't much more than that.
