I know this festival in NL has a mobile tattoo studio, the health department people (dunno what the exact term for those is in English) checked it and approved, coz they could keep it as clean as a normal studio

They can keep the mobile studio clean, yep.
But I doubt I could keep my new tattoo clean on a festival like Wacken - all the dust and mud and sweat and heat...guess it would definitely hurt more and possibly lead to an infection...and drinking alcohol before/after being tattooed isn't ideal either.

Why would it hurt more, hurt is a personal thing. It depend on your pain threshold and where on your body you get the tattoo done.
Not where in the world you get it done.
Pain in a normal tattoo studio in Japan is the same as pain in a german caravan kept totally clean somewhere on a meadow on a festival

and if it is a good/responsible tattoo artist he/she would not tattoo somebody who is drunk
drinkin after getting a tattoo done is your own responsibility

Not the tattooing procedure itself would hurt more, but I just imagine* it's easier to cope with a fresh tattoo at home at 20°C than in the sun at Wacken at 35°C, and I guess it's harder to keep it absolutely clean. If we're talking about a person who can ignore pain and who is responsible, well, why not. But still I think a normal studio would be a better setting than a festival.
