I don't know what real Paladins were, but in every game they appear in (D&D, Warcraft, etc.), they're usually some combination of a knight and a cleric. I imagine it's something from the Crusades...

It actually means "belonging to the roman mountain Palatin" (and therfore to the royal palace. The special guard of Charlemagne were "the twelve paladins" (Amongst them Roland, Olivier, Archbishop Turpin), also we are not on historic ground there anymore. The word was used throughout the middle Ages for for very loyal followers, mostly the royal guards.
